PAVE -- Partnerships for Action, Voices for Empowerment provides information and support to families of children with disabilities and special healthcare needs. The Family Resource Coordinator will guide the family through the process of eligibility determination, setting up a meeting to write an Individualized Family Service Plan (IFSP), assuring needed services are provided and helping the family transition to a School District or other needed services at age three.
Minimum requirements:
• Understanding of the State Birth-to-Three requirements for infants and toddlers with developmental delays and disabilities in Washington State
• Understanding of IDEA Part C requirements and awareness of the new Part C Regulations
• Ability to identify strategies for referral and assistance for families in scheduling evaluations in the areas of concern and discuss service provider options
• Ability to coordinate with entities within the County, local lead agency (LLA), Service Providers for Birth-to-Three services, and local school districts on behalf of families
• Coordinate and work with the family to develop the initial Individualized Family Service Plan (IFSP) meeting when a child is eligible for services, and subsequent reviews
• Ability to assist families with their application for funding assistance, as needed, to include a basic understanding of what those funding mechanisms may include
• Ability to maintain and ensure case records are clear, accurate, and professional and maintained so that family confidentiality is assured
• Ability to develop partnerships with other FRC within the County to assure families receive all needed services
• Proven computer skills using Microsoft Office and ability to utilize database systems
• Demonstrated excellent verbal and written communication skills
Other skills desired:
• Experience with data input and collection strategies
• Noted experience in working within Washington State and/or Pierce County to help families access care and services for infants, toddlers, and youth with disabilities or developmental delays
• Ability write with clarity and measurement when considering possible goals for a child with disabilities
• Multi-lingual skills
• Knowledge/experience with military systems of care
