Outreach Worker to monitor and support access to dental and primary medical care. Schedule and arrange dental care and monitor hygiene care services for patients, and to recruit patients for healthcare.


COORDINATES WITH Pierce County outreach sites providing services for persons experiencing homelessness to include schools, family support centers, meal sites and other services centers. May coordinate with special events such as the Annual Homeless Connect and oral health collaboratives.

MINIMUM QUALIFICATIONS:
Postsecondary training certificate in healthcare or closely related social services field and three (3)
years of experience providing direct care to the target homeless population. The incumbent must pass a criminal background check which may include fingerprinting. Must provide and maintain on file a copy of current first aid and CPR card. Due to the nature of this position the incumbent must have reliable personal transportation.

DESIRABLE QUALITIES
-Strong commitment to persons who are experiencing homelessness
-Excellent conversational, diplomatic, public relations and cooperative skills.
-Capable of strict code of ethics, confidentiality and boundary setting.
-Ability to pay close attention to detail and to follow established recruitment procedures and medical
HIPAA guidelines.
-Knowledge of interviewing in outreach and office setting.
-Ability to maintain confidential, professional communication.
-Ability to gain trusting communication with youth and adults who lack housing or healthcare.
-Knowledge and ability to collect, analyze, evaluate and report data accurately.
-Knowledge and skill in case work methods.
-Knowledge and ability to successfully implement intake interviewing and case management.
-Knowledge of Tacoma and Pierce County area.
-Knowledge and ability to refer to entitlement programs
-Working knowledge of computers and computer data entry processes, or the ability to learn
-Knowledge of and ability to access community social, medical , mental , chemical dependant services, housing employment and other needed resources as appropriate based on client need.
