West Sound YFC is seeking 1 full-time (40+ hours per week minimum) Foster Care Case Manager. Per contract requirements, the Case Manager must have a bachelor's degree in social work or related field. Case Manager Job Description

Main Goal: Implement the case plan established by the courts and professional team for foster children on your individual caseload.

The Case Manager oversees foster children placed in YFC foster homes to ensure these placements serve the best interest of the children and conform to legal and contractual requirements. The Case Manager is additionally responsible for monitoring the child's physical, emotional and spiritual well-being.

Key Result Area #1: SUPPORT FOSTER CHILDREN IN THEIR PLACEMENT
Supporting Goal: Offer services and activities to support foster children in their placements.

Performance Standards:
A. Advocate on child's behalf with schools, courts, mental health systems, and other agencies -- maintaining standards of confidentiality.
B. Coordinate with the YFC Visitation Manager to schedule and/or supervise parent/child and sibling visitation while maintaining open communication with all parties
C. Support foster families as they deal with the demands of foster parenting including education pertaining to the foster care system.
D. Provide 24-hour availability to assigned foster families as needed.
E. Coordinate with the professionals involved with each child's case plan.

Key Result Area #2: MEET CONTRACTUAL REQUIREMENTS
Supporting Goal: Ensure contractual requirements are fulfilled for a caseload of foster children.

Performance Standards:
A. Develop a Safety and Supervision Plan for each foster child on your case load and update the plan every six months.
B. Perform Health and Safety Visits every 90 days as required by our contract.
C. Provide written documentation of child's progress through regular narrative updates (case notes), quarterly reports and other communication as appropriate and required.
D. Maintain current information in foster child client files and notify social workers and foster families of pending needs in a timely manner.

Key Result Area #3: PROFESSIONALISM
Supporting Goal: Promote a positive image of West Sound YFC in our community.

Performance Standards:
A. Work effectively and maintain positive and healthy relationships with social workers, Guardian ad Litems, therapists, school personnel, foster parents and others to achieve the goals set forth in the child's case plan.
B. Pursue continuing education and training to improve effectiveness and knowledge of requirements in the position of Foster Care Case Manager
C. Immediately report incidents of concern to the Foster Care Director or Executive Director

Key Result Area #4: ADDITIONAL YFC RESPONSIBILITIES
Supporting Goal: Further the mission of Youth for Christ.

Performance Standards:
A. Effectively communicate the mission of YFC in the community.
B. Active participation in West Sound YFC's ministry of development.
C. Adhere to the YFC Ministry Site Essentials and WSYFC Core Values.
