F/T Housing Case Manager (Auburn, WA): SUMMARY OF RESPONSIBILITIES:

The Coming UP Case Manager provides intensive Case Management services to 15 youth ages 18-24 in rental facilities funded by Section 8 Vouchers. This program, titled "Coming Up" is a collaborative effort between AYR, Kent Youth and Family Services, and Valley Cities. This position requires travel to the rental units as well as transporting clients to services or appointments as needed. The Facility Based Case Manager works as a member of AYR's Outreach program and works closely with the Outreach workers, peer outreach workers, non facility case managers, and reports to the Outreach Program Coordinator. The position requires non traditional work hours, crisis intervention, mentoring and advocating for youth in this program while modeling and teaching life skills and appropriate behavior. The Facility based Case Manager establishes a positive rapport with the formerly homeless youth they have enrolled. The Outreach Case Manager sets up appropriate treatment plans and makes appropriate referrals while establishing clear and appropriate boundaries with youth served.

ESSENTIAL FUNCTIONS:

1. Provide direct service to youth with a focus on under served and hard to serve populations.
2. Act in the role as "Advocate" to youth for the entire duration of their participation in the "Coming Up" program.
3. Provide case management services as needed to "Coming up Youth".
4. Maintain data collection and case records as required by the grant and/or the contract.
5. Maintain client confidentiality at all times.
6. Develop and maintain positive working relationships with Kent Youth and Family Services Counseling and Drug and Alcohol liaison, as well as the housing Coordinator and peer specialist from Valley Cities.
7. Be familiar with youth-serving programs throughout King County.
8. Work with the Outreach Coordinator to develop and update resource information for the King County region.
9. Communicate regularly with the Outreach Coordinator about ongoing program development and implementation areas of concern as they arise.

OTHER FUNCTIONS:

1. Participate in outcome evaluation tasks as dictated by funding source requirements.
2. Participates in Outreach Staff meetings and "Coming Up" staff meetings weekly.
3. Prepares reports and participates in special projects (such as presentations, trainings, etc.) as assigned.

REQUIRED EXPERIENCE:
1. Highly effective in working objectively with a diverse group of people and must demonstrate communication, organizational, administrative and office managerial skills.
2. Must be a team player willing to accept and promote organizational goals and function with minimal supervision.

EDUCATIONAL REQUIREMENTS:

1. Bachelor's Degree in human services field (i.e. Social Work, Psychology) from an accredited college or university. 2-3years of experience working in community mental health preferred. Relevant experience may substitute for education.

REQUIRED LICENSES, CERTIFICATES OR KNOWLEDGE:

1. Current Washington State Registered Counselor Status.
2. Current Washington State driver's license and proof of valid insurance.
3. HIV/AIDS training.
4. Proof of negative TB test
5. Current CPR/First Aid certificate.
