Case Manager for residential teens - New Vision Programs (Lacey)

Case Manager for residential teens (Lacey): New Vision Programs is a growing social services agency in Oregon and Washington. Our philosophy incorporates an individualized approach to working with behaviorally challenged youth. We are seeking a candidate with strong clerical and organization skills to work in a fast paced environment at both of our Washington locations in Lacey and Vancouver:

PURPOSE:

1.To develop an appropriate/efficient individual service plan for each resident and participant of New Vision Programs that reflects our philosophy, policies and procedures.
2.To organize and maintain paper files and electronic files on a Windows based PC and utilizing current MS Office software
3.Encourage the spirit of teamwork, hospitality, confidentiality, and organization within the flow of the office and programs of New Vision Programs.
4.Communicate with Guardians and work with each client to develop realistic objectives for the period of residence at New Vision Programs and for transition back into the community.
5.Coordinate appointments working cooperatively with the program director, foster parents, and residential care facility staff to develop a comprehensive plan for each client.
6.Establish and maintain accurate and timely written records of client case plan, notes and progress. Filing the records in an orderly manner.
7.Coordinate periodic assessment of each client's progress toward achieving the agreed objectives in case plan.
8.Inform and keep program director, foster parent and/or residential care facility staff of current progress of case plan.
9.Complete initial service plan and master plan; 30, 60 and 90 day reviews; discharge and discharge summaries.
10.Coordinate services with educators, psychologist, therapists, caseworkers, medical and dental personnel, house managers, New Vision Programs staff and other service providers.
11.Maintain current information manual regarding community resources and to work effectively with such resources to obtain services for clients.
12.Transport clients for services as needed including scheduled and unscheduled appointments.
13.Ensure the implementation of the "team approach" by participating in and/or attending all staff meetings, including by not limited to, executive staff meetings, house meetings, team meetings, retreats, and events.
14.Ability to work with people of diverse ethnicity, socio-economic circumstances, religion, culture and sexual orientation with dignity and respect.
15.Bachelors of Arts (B.A.) degree in social science or related field. A minimum of four (4) years experience working with children and families in lieu of degree. New Vision Programs considers education, training, experience and demonstrated skills.
16.Demonstrated understanding of appropriate activities for physically, intellectually and emotionally and developmentally challenged adolescent youth.
17.Demonstrated ability to develop effective case plans, timely case notes and maintain accurate written records.
18.Demonstrated ability to collect and utilize information regarding community resources and to work effectively with such resources to obtain services for clients.
19.High tolerance for ambiguity and change.
20.Ability to type/word process; use electronic mail; develop documents such as letters, memorandums, presentations and reports using Microsoft Office Products.
21.Skill/ability to effectively communicate (oral and written).

OTHER REQUIREMENTS:

1. Position requires personal transportation. If driving a personal vehicle, a valid license for the state of residency, and a driving record permitting coverage under corporate liability policy.
2. Must pass a criminal history check.

Requirements: Passion to help kids progress and achieve self efficacy. Bachelor's Degree, outgoing, people friendly, energetic, public speaking experience, good writing and organization skills.
