P/T Outreach Worker / Case Manager (Auburn, WA): SUMMARY OF RESPONSIBILITIES:

The Part time Outreach Case Manager establishes a positive rapport with the homeless youth while interacting with them on the street and in the community with the intent of engaging them in further case management services. In the street or community setting, the outreach worker/case manager will build a relationship while setting up appropriate treatment plans and making appropriate referrals based on the goals of the client -- including but not limited to: housing, basic needs, employment, education, legal services, medical care, and child care. The Outreach Case Manager establishes clear and appropriate boundaries with the youth served.

ESSENTIAL FUNCTIONS:
1. Provide direct service to youth with a focus on under served and hard to serve populations.
2. Act in the role as "Advocate" to youth.
3. Provide case management services as needed to eligible youth.
4. Maintain data collection and case records as required by the grant and/or the contract.
5. Develop linkages with community agencies including schools and police as needed.
6. Be familiar with youth-serving programs throughout King and Pierce County.
7. Work with the Outreach Coordinator to develop and update resource information for the King County region.
8. Communicate regularly with the Outreach Coordinator about ongoing program development and implementation areas of concern as they arise.
9. Partner with Peer Leaders.
10. Utilize the expertise of youth in day-to-day provision of services to hard to reach or hard to engage homeless youth.
11. The Outreach Worker / Case Manager works well within the team demonstrates positive support for colleagues and works to reduce conflict.

OTHER FUNCTIONS:
1. Participate in outcome evaluation tasks as dictated by funding source requirements.
2. Participates in clinical staff meetings, as appropriate.
3. Prepares reports and participates in special projects (such as presentations, trainings, etc.) as assigned.

REQUIRED EXPERIENCE:
1. Highly effective in working objectively with a diverse group of people and must demonstrate communication, organizational, administrative and office managerial skills.
2. Ability to work with ,and engage youth in the streets.

EDUCATIONAL REQUIREMENTS:
1. Bachelor's Degree in human services field (i.e. Social Work, Psychology) from an accredited college or university. Two-three years of experience working in community mental health preferred, and may substitute for educational requirement.

REQUIRED LICENSES, CERTIFICATES OR KNOWLEDGE:
1. Current Washington State Registered Counselor Status.
2. Current Washington State driver's license and proof of valid insurance.
3. HIV/AIDS training.
4. Proof of negative TB test
5. Current CPR/First Aid certificate.
